Terms and Conditions for Mercedes Cusick Therapy Services
Please note: All clients receive a comprehensive informed consent document prior to beginning therapy. The information below is a general overview of our practice policies.
Scope of Services
Mercedes Cusick Therapy Services offers psychotherapy services provided by Licensed Marriage and Family Therapist, Mercedes Cusick, LMFT. Services are available to individuals seeking support for trauma, mental health, and relationship concerns. Therapy is tailored to each client’s unique needs using evidence-based, trauma-informed approaches.
Confidentiality
Your privacy is a top priority. All sessions and communications are kept confidential in accordance with California law and the ethical standards of the profession. However, confidentiality may be legally broken under the following circumstances:
- Imminent risk of harm to self or others
- Suspected abuse or neglect of a child, elder, or dependent adult
- Legal subpoena or court order
- Other situations as required by law
If a situation arises that requires breaking confidentiality, we will make every effort to discuss it with you first whenever possible.
Client Responsibilities
Clients are expected to actively participate in the therapy process, provide accurate information during sessions, and attend appointments on time. A minimum of 24 hours’ notice is required for cancellations or rescheduling. Late cancellations or missed appointments may incur the full session fee.
Payment and Fees
Payment is due at the time of each session unless other arrangements have been made in advance. Accepted forms of payment include Zelle, Venmo, Credit/debit cards, Cash, or Check. A fee agreement outlining current rates and policies is provided before the start of therapy.
Records and Documentation
Mercedes Cusick maintains secure and accurate records of client information in compliance with HIPAA and professional guidelines.
Termination of Services
You may choose to end therapy at any time. Likewise, Mercedes Cusick reserves the right to discontinue services if it is determined that therapy is no longer beneficial or appropriate. Referrals to other providers may be offered when needed.
Emergencies
This practice does not provide 24-hour crisis services. In case of an emergency, please call 911 or go to your nearest emergency room. You may also contact a local crisis line or national support resource. If needed, Mercedes can assist with locating appropriate crisis services.
